Powerful Tools for Caregivers
Purpose:
Provide family caregivers with tools to increase their self-care, caregiving ability and confidence.
Background:
Based on Chronic Disease Self-Management Program developed by Dr. Kate Lorig and colleagues at Stanford University.
Developed under Meyer Memorial Trust to Legacy Health System, Portland, Oregon
Revised by Mather Institute on Aging with funds from the National Family Caregiver Disseminated nationwide by Legacy Caregiver Services and revised in 2007.
Audience:
Any adult who is a caregiver.
Class Leaders:
Two licensed leaders co-teach each series.
Format:
Six 90 minute sessions held once a week or
Six two and one-half hour sessions held once a week.
